Few longitudinal follow-up studies have investigated the association between level of education and adherence to multiple health recommendations. Moreover, few studies have examined the socioeconomic distribution of the potential loss of life expectancy associated with non-adherence to health recommendations.
The aim of this thesis is to study the longitudinal relationship between level of education and adherence to health recommendations regarding smoking, physical activity, body mass index and alcohol intake using follow-up data of adults living in Tromsø, Northern Norway. Furthermore, the socioeconomic distribution of the potential loss of life expectancy that might be associated with non-adherence to the health recommendations is estimated using cross-sectional data.
Article 1 investigated the association between level of education and patterns of adherence to the health recommendations. Data from adult participants with repeated measurements were drawn from the sixth and seventh waves of The Tromsø Study.
Article 2 examined the association between level of education and adherence to the health recommendations. Logistic regression estimated the relative educational inequality in the adherence to health recommendations at the baseline and follow-up measurement points.
Article 3 explored the association between level of education and income with adherence to health recommendations using data from Tromsø 7. Adherence rates across education and income were estimated, and published life-expectancy loss estimates were combined with the participants’ behaviors to estimate potential years of life lost.  The socioeconomic distribution of the potential loss of life expectancy that may be associated with non-adherence to the health recommendations was estimated by conducting probabilistic sensitivity analyses using Monte Carlo simulation.
There are clear socioeconomic differences in the adherence to the health recommendations regarding smoking, physical activity, body mass index and alcohol intake. Higher levels of education were found to be associated with adherence to the recommendations regarding smoking, physical activity and body mass index, and with non-adherence to the recommendations about alcohol intake. The relative educational disparity in adherence to health recommendations seems to widen as people age and the socioeconomic inequality in adherence could lead to larger socioeconomic differences in life expectancy.
