The medical case report (CR) is one of the oldest medical genres and it is still popular among clinicians. At the end of the 20th century medical journals gave priority to research articles with more advanced study designs. Following the advent of online publishing, a high number of CR are now being published. We identify the various merits and limitations of the CR genre. Two important merits are the CRs ability to draw attention to new observations and the ability to present new hypotheses. The CR is also an important tool for teaching and learning clinical medicine. It has limitations, including that it cannot produce epidemiological quantities or make causal inferences. In a bibliometric, descriptive study of all medical scientific articles produced in a decade at the University Hospital of North Norway, we found only 2.2% CR. In three articles we illustrate the working up and format of modern CRs and demonstrate how CRs can contribute to the scientific literature.
